Open cut coal mine

Summary: A fitter was standing on a work platform attaching a lifting sling to a radiator cowling. The cowling was not adequately restrained and fell forward, causing the ladder and fitter to fall. The fitter landed on a nearby tool trolley and was uninjured.

Comments to industry: When conducting maintenance activities involving the removal of objects, controls must be put in place to manage the risk of the objects falling. Mine operators should ensure that pre-use inspections are available for workers and are completed for all lifting/lowering equipment tasks. The rating of all lifting components within a system should consider the worst-case scenario where the load becomes unbalanced.

Uncontrolled movement of loader
A front-end loader drove up a ramp on a stockpile. The loader was on a soft edge as he backed down, so he stopped immediately and asked for help. The operator had recently commenced employment at the site, had previous loader experience, and was signed off as competent on the loader after observation and training.

Tyre failure ejects rock through dozer cabin - Safety Alert #392
As a truck has pulled away from the excavation face, one of its tyres ran over a large piece of rock (Basalt). This caused an immediate tyre failure, which in turn caused rock shrapnel to eject from the floor of the area, hitting a dozer more than 40 metres away.
