Dangerous incident | IncNot 0035223
Summary: An operator's gloved hand became wrapped around a drill steel, dislocating two fingers. The incident occurred when the worker was in the process of drilling a hole using a hand-bolter on a longwall monorail bolting platform.
It was reported that the operator’s foot accidentally triggered the rotation of the steel through contact with the bolter trigger, wrapping his hand and glove up in the rotating steel.
Recommendations to industry: Mine operators should review risk assessments and procedures in relation to working in close proximity to rotating components, giving proper consideration to the risk of entanglement from clothing and gloves.
Serious injury | IncNot 0035231
Summary: A fitter suffered a back injury after lifting his tool bag onto the tray of a utility while working underground. The bag was subsequently weighed and found to be more than 30 kilograms. The worker lost consciousness momentarily and was in significant pain. He was transported to the surface and by ambulance to the hospital for treatment.
Recommendations to industry: Following the incident, the mine operator has issued a directive that tool bags should not be more than 20 kilograms.
Mine operators should have processes in place where supervisors routinely monitor the manual handling practices of maintenance personnel, particularly mechanical trades who are frequently expected to handle heavy loads as part of their duties.

Stop use and product recall - 3M twin leg self-retracting lifelines
3M Fall Protection has announced an immediate stop use and product recall of the 3M™ DBI-SALA® Nano-Lok™ edge and Wrap Back Twin-Leg Self Retracting Lifelines. The twin-leg Nano-Lok edge is part of a personal fall protection system and connects two self-retracting lifelines/devices (SRLs/SRDs) directly under the dorsal d-ring of a worker’s harness. It is intended to be anchored at foot-level and is designed for sharp edge applications.

Since early September 2018, MSHA has documented nine equipment fires involving Hitachi excavators and shovels and Caterpillar haul trucks, front-end loaders and dozers.

Truck rolls over while spreading material
A truck, with its body raised, was spreading material on a quarry haul road, when the operator misjudged the road camber and became concerned with the lean of the truck body.
The driver tried to lower the hoist to reduce the lean as he continued to spread metal, but the centre of gravity had changed sufficiently to the left to snap the mount of the lift ram and cause the truck’s body to come to rest on its side against the batter. No-one was injured in the incident.
