Summary: A haul truck was travelling adjacent to an office complex and car park. The position five tyre failed, causing stones to be thrown towards the office complex and car park. A window was broken and a golf ball-sized rock landed near a person in the car park. There were no injuries.

Summary: A worker was driving a light vehicle on an access road when the steering failed. The vehicle veered left and came to rest in an adjacent drainage channel. The vehicle was travelling about 40 kph. The worker was uninjured.

Comments to industry: The operating environment of vehicles must be considered when developing maintenance strategies for component inspection and replacement.

Summary: A storm front moved over a mine resulting in more than 100 mm of rain falling within 30 minutes. A creek between the open cut pit and dump burst its banks and flowed into the pit. No workers were in the mine at the time.

Summary: A mobile processing unit (MPU) was transported offsite for maintenance. Three days later, an inspection of the pump cabinet at the maintenance facility offsite identified emulsion was present. Inspections identified half a tank of emulsion and half a tank of gassing agent where still onboard. No AN locks were in place on the tank or cabinet. The truck was returned to the mine.

Comments to industry: This incident is being investigated by SafeWork NSW. However, this incident is a timely reminder for mine operators to have a robust system in place to ensure all equipment is fit to leave the site.

Mine fatality – On 17 November, 2021, a customer truck driver was electrocuted after the tarping mechanism on the trailer made contact with a high-voltage overhead power line. While exiting the cab of the truck, the worker made contact with the energised truck and suffered a non-fatal electrical shock. When he tried to re-enter the cab of the truck, he was electrocuted.

Safety Alert #15-2021 Valve falls from height striking and seriously injuring a worker - While reciprocating casing during a cement job on a drill rig, a valve weighing greater than 15 kg broke off at the threaded nipple union and fell approximately 5 to 6 metres, striking a worker in the leg and causing serious injury.

SafeWork SA is issuing a safety alert to remind PCBUs that they must not direct or allow a worker to carry out work activities without holding the relevant high-risk work licence following an incident involving a vehicle loading crane.
