Dangerous incident | IncNot 0035080
Summary: A truck was refuelling at a fuel bay on the surface of an underground mine. Fuel splashing from the breather contacted hot engine parts, initiating a small fire.The operator manually triggered the automatic fire systems and extinguished the fire. The refuelling system was a dry break system. No-one was injured.
Recommendations to industry: The risk of strata failure should always be considered, with appropriate controls identified and implemented before work is undertaken in such locations.

Summary: An unattended light vehicle moved an estimated 250 metres from where it was parked. It hit a windrow and flipped onto its side. The vehicle had been parked behind a drill rig but rolled away when unattended.

Recommendations to industry: Drivers must comply with site protocols for vehicle park-up before leaving their vehicles.
Dangerous incident | IncNot 0035103
Summary: Two people were working on an elevated working platform (EWP) replacing wall sheeting at a coal handling preparation plant. One of the operators tried to move the EWP upwards but the machine moved unintentionally and hit the underside of a structure. No-one was injured.
Recommendations to industry: The investigation has found that the control mechanism of the EWP malfunctioned. The cause of the malfunction is yet to be been determined.
