Underground coal mine 
Summary: Workers were installing timber props in an underground roadway. Following the installation of one prop, the workers noticed that it was not vertical and decided to fix the alignment. One worker was hammering a timber wedge at the base of the prop when a wedge at the top of the prop fell and struck the worker on the head and neck.
Comments to industry:  Mines must identify new hazards that may be introduced by changes to traditional work practices and assess the risk associated with the hazard. If there is a risk of heavy objects falling into a work zone then appropriate controls must be put in place to protect workers from injury.
