DD Council feedback for an environmental scan is needed!
The Administration on Community Living recently awarded a 5-year grant to create a National Health Care Transition Resource Center (NHCTRC) for Youth and Young Adults with ID/DD.
Family Voices is the principal investigator and Got Transition, SPAN, University of Missouri-Kansas City, and the Waisman Center are partners in this effort.
A major activity of this grant is to create a new, accessible website.
Got Transition is conducting an environmental scan to learn about HCT initiatives and existing health care transition (HCT) resources around selected topic areas that can assist youth and young adults with ID/DD during the transition to adult health care.
To help compile these resources, please fill out this online survey with information about your existing resources and initiatives.
