Global letter of support for CPC+ practices

A new CMS federal program will allow primary care practices to receive payment incentives in the Comprehensive Primary Care Plus initiative. The deadline for application is September 15. This is a regionally-based multi-payer payment reform and care delivery transformation. Ohio and Northern Kentucky comprise one of 14 eligible regions. Only 5,000 practices will be chosen nationally. The program allows primary care practices to apply for two different tracks for incentives. Track 2 is more rigorous than the first track. As part of the application process for Track 2, practices need to provide letters of support from HIT vendors. While you will need a letter of support from your EHR vendor, you also can use CliniSync HIT as a supplement to assist you in achieving your requirements. 

HIE Services to Supplement CPC+ Track 2 Practices
  • Results Delivery -  Practices can receive patient lab results, radiology and transcribed reports directly into their EHR, such as care summaries, history & physicals, and progress notes.
  • Community Health Record – Practices can access a single community record for patient encounters with other CliniSync hospitals and providers, including related results and reports.
  • Contribute -  Practices can contribute care summary documents or care plans to CliniSync for other providers to access.
  • Referrals – Practices can electronically send and receive patient referrals with confirmation and tracking in real time, which is especially helpful when coordinating with social service organizations.
  • Notify -  By uploading patient panels, practices can receive a notification when one of their patients goes to the ER or is admitted to or discharged from the hospital.
  • CliniSyncPLUS – Practices can sign up for consulting services to help them prepare to meet the requirements of the CPC+ program including but not limited to assistance with risk-stratification, documentation requirements, ensuring required EHR interoperability functions are enabled, workflow analysis and re-engineering. This team can assist with both Track 1 and Track 2 requirements.

We're holding a webinar on Wednesday, 8/24 for physician groups and hospitals on CPC+ entitled: Two New Reimbursement Models: CMS and Medicaid

This webinar will provide an overview of the requirements, advantages, challenges and registration process for the CPC+ program that was recently announced for Ohio.  We will also cover the similarities and variances of the Medicare and Medicaid CPC+ programs.

The Centers for Medicare & Medicaid Services (CMS) will host two webinars in August that highlight how to participate successfully in the EHR Incentive Programs in 2016 based on the criteria outlined in the October 2015 final rule. The first webinar will discuss criteria for eligible professionals (EPs) and the second will highlight requirements for eligible hospitals and critical access hospitals (CAHs). 8/24 CliniSync User Group online meeting

If you are a CliniSync member, please join us for our second online CliniSync User Group meeting on August 24 from 11 a.m. until noon. We hope staff members within your organization can listen in. We’re interested in hearing from hospital staff as well as clinicians in practices working with CliniSync.
 
Our agenda includes a presentation by John Leite of the Healthcare Collaborative of Greater Columbus on the "medical neighborhood" concept now taking place in Central Ohio. Steve Thompson of Helping Hands Health and Wellness Center in Columbus will explain how this free clinic is reaching out to other community providers using CliniSync tools. Adam Rossbach, our HIE Director, will be presenting our newest service – Notify – and will update you on our progress.

Notify allows practices to submit a patient panel and then be alerted when one of those patients is admitted or discharged from the Emergency Department or the hospital. While we don't yet have a demo of its functionality, Adam will be presenting information on it.
