HIT doesn't take a vacation!  At 11 a.m. August 24, Wednesday, we will hold the second CliniSync User Group online meeting to discuss topics of interest to our members. 

We will be exploring new products and want you to share your best practices. Helping Hands Health and Wellness Center in Columbus will share how their free clinic reaches out to community providers and partners using CliniSync tools. Adam Rossbach, our HIE Director, will discuss the debut of NOTIFY, a new solution that will allow physicians and others to receive a notification when a patient is admitted or discharged from the Emergency Department or hospital.

Best practices: How a large group practice manages their chronic care patients

We've worked with Westshore Primary Care and the Ohio Department of Health to bring you best practices via webinar and an article on how this large group practice manages their chronic care patients. Comprised of 60 physicians in Cuyahoga and Lorain counties, this practice has begun to focus on patients with chronic conditions, such as diabetes, who have  been released from the hospital and require follow-up care.

The webinar and article, Establishing a CCM Program in an Independent Group Practice are part of an ongoing series on Improving Ohio's Health through managing the care of patients with diabetes and hypertension. 

What did the practice learn?

"We knew we wanted to work with patients who were recently discharged from the hospital to prevent their re-hospitalization. Working with them on an ongoing basis through the CCM has created the bridge between visits that helps us help them. It allows us to go a lot deeper into our patients’ social needs—why does a patient miss an appointment and how can we help? That way we’re addressing more than just their medical issues.The improvement in the continuity of our patients’ care has been enormous."

Our comments on MACRA

While there's discussion of a proposed change to MACRA legislation in Congress we wanted to make sure you know that we submitted comments on behalf of our CliniSync Community to the Centers for Medicare and Medicaid Services on June 24. MACRA is the acronym for the Medicare Access & CHIP Reauthorization Act of 2015. To provide CMS with comments on how the proposed rule would impact our statewide users, we invited a group of stakeholders to share their input and expertise. Through a series of calls, we developed the comments that were submitted to CMS.
