MESSAGE FROM THE CHAIR

For those of you who attended the Gallie Day Scientific Program on May 8, 2015, you will recall the outstanding symposium that was held on “Big Data”.  For this symposium, we heard perspectives from Geoffrey Anderson from the Department of HPME on the use of big data for discovery research; from Nancy  Baxter on harnessing the power of big data for health research; from Steve Gallinger on the utility of big data for cancer genetics research; and from Avery Nathens on using big data to study injury prevention in a population of patients who had undergone bariatric surgery.  Everywhere you turn these days, you seem to bump up against the term “big data”.  But what does it mean?  Big data can be defined as extremely large data sets that may be analyzed computationally to reveal patterns, trends, and associations, especially relating to human behavior and interactions.  Recently, the American College of Surgeons (ACS) Bulletin had an informative article on the subject of big data (1).  For us as surgeons, we are of course keen to query our hospitals’ electronic medical record (EMR) to track the care of our patients so that we can ultimately improve upon quality of care.  One of the problems, however, has been the proliferation of EMRs, and the time taken, especially for our residents and fellows, to train on different ones during different rotations.  I was delighted to learn of the number of hospitals at UofT that are now collecting patient data for the National Surgical Quality Improvement Program (NSQIP).  NSQIP has been used time and again to improve the delivery of quality care to surgical patient populations as we learned from the Gordon Murray Lecturer this year at Gallie Day, Dr Clifford Ko.  In the ACS Bulletin article on big data, the authors conclude that surgeons should be involved in supporting big data ecosystems.  This can take the form of surgeon-leaders who are involved in ensuring that the correct “fields” of data are captured in every large database; and surgeons who will assist with the design of “smart apps” which identify moments in patient care that can be tracked and on which research can be performed; and avoiding data silos which by their very nature limit the power of the big data opportunity.  As one of the most important facets of big data clinical registries is to advance the quality agenda, and as quality and best practices are a major pillar of our strategic plan in the Department of Surgery, I do hope all of you are becoming more involved in the practice of entering data on your patients in registries of one type or another, and using the appropriate research tools to study your patients’ outcomes after surgical procedures.  As the adage goes, “there is strength in numbers”.

David Urbach and Nancy Baxter (General Surgery) are the recipients of the 2014-15 CIHR-IHSPR Article of the Year Award. This award recognizes published research that has significantly contributed to the advancement of the field of health services and policy research in Canada.  The article is entitled “Introduction of Surgical Safety Checklists in Ontario, Canada ".  In addition to receiving an award of $10,000, Drs.
