MESSAGE FROM THE CHAIR
|As surgical educators, one of our most important tasks is to ensure that the graduates from our training programs are ready to meet the challenges and demands of a career in surgery. We had always taken great pride in believing that our graduates from the Department of Surgery at the University of Toronto were a cut above the graduates from other training programs, and that our trainees were “practice ready” upon completion of their residencies. Interestingly, there has been a tremendous evolution in the surgical specialties over the past decade in Canada characterized by increasingly complex surgical cases, the advent of sophisticated intra-operative technologies, and a clearly identified and articulated specialist underemployment phenomenon(1) such that nowadays virtually all of our graduating residents are pursuing additional years of training as fellows to be fully prepared to practice as independent surgeons. This is why I was very interested to read the article in the New York Times by Pauline Chen(2) from December 12, 2013 entitled “Are today’s new surgeons prepared?”. In the article Chen refers to a report published in the Annals of Surgery which describes the inadequate preparation of graduating US surgical residents entering fellowships(3). In this report, fewer than half of the surgeons were evaluated to operate or to make accurate clinical decisions on their own; and one third were unable to perform a laparoscopic cholecystectomy. While the reasons for the potential decline in competency of surgical residents at the time of graduation is most likely multifactorial, at least one of the factors identified by this report was a reduction of time spent in the operating room during residency, and a loss of approximately 1 year of operative training because of the restrictions in resident duty hours in the US. It is clear that surgical thought leaders do not wish to return to the 100-plus hour work week for trainees. Rather, surgical training must be made more efficient than ever before, and time on task must be made to count at every opportunity on the ward, in the emergency room, and in the operating room. That is why I applaud the efforts of our Division of Orthopaedics for their promulgation of a Competency Based Curriculum (CBC), a curriculum that we are most eager to emulate across all Divisions within the Department of Surgery. In this regard, and working with our leaders in Surgical Education in the Department, we are making tangible progress in this direction as an important part of our Strategic Plan.
