MESSAGE FROM THE CHAIR

2nd Annual Global Surgery Symposium

With Global Surgery emerging as a significant career path for our residents and faculty, symposia for sharing experiences and strategies become increasingly important to our Department. After a well-received inaugural Global Surgery symposium last year, we wanted to hold the event once again this year, but with a slightly different focus. For those who attended last year, you will recall that several of our faculty shared their stories and lessons learned abroad in an open forum. This year, we decided to narrow our focus towards the role of partnerships and teamwork in building a surgical program in low income countries.  This approach coincided with the findings of the recent Lancet Commission publication on Global Surgery* which aimed to provide an ongoing strategic approach to providing accessible and high-quality surgical care to disadvantaged regions.

We could not have been more fortunate.  Anna Dare, resident in the Division of General Surgery, was an investigator on this document, and was mentored in part by one of the Lancet Commission’s chief authors, Dr. Andy Leather from King’s College in London UK.  Several emails and committee meetings later, we were honoured that Dr. Leather agreed to join us as our keynote speaker for the Global Surgery Symposium held on February 17th, 2017.

The event began with Dr. Leather offering a first-hand look through his journey of building a program to enhance surgical capacity in Sierra Leone. His overarching message of “Go slow and go low” applied to all subsequent presentations: We simply cannot rush the process of establishing trust and gaining local respect when bringing our assistance abroad, as the institutional memory of what goes into long-term collaboration will ultimately determine its success. To this effect, community advocacy will often be just as important as clinical education in these resource-deprived countries.

Our own faculty spoke next, echoing these lessons and speaking to their personal experiences. Dr. Rajiv Singal (Urology, the Michael Garron Hospital) spoke of his involvement with the Zomba Hospital in Malawi. He discussed medical access as being embedded in a country’s culture, and the challenge of trying to change culture at a systems level. He emphasized that not every meaningful change requires skill or money, as sometimes the challenge lies in the absence of administrative support or basic supplies such as running water. Dr Miliard Derbew, President of COSECSA, and a pediatric general surgeon at Addis Ababa University, spoke of the success of the involvement of numerous Department of Surgery faculty at UofT, in surgical efforts in Ethiopia.  Dr. Ted Gerstle (General surgery, Sick Kids) spoke about his involvement with the Toronto Addis Ababa Academic Collaboration with a paediatric perspective. Dr. Georges Azzie (General surgery, Sick Kids) presented a thought-provoking perspective on the definition of global success, and whether the metrics of success are defined by one’s academic institution or the hosting country’s health measures. Dr. Oleh Antonyshyn (Plastics, Sunnybrook) took us through the surgical response to the recent war crisis in Ukraine. A multi-discipline team was able to treat injuries stemming primarily form blunt force traumas, craniofacial and globe trauma. He also discussed how the purchasing and generous donation of equipment from Canada allowed for Centers of Excellence to be established.

I was absolutely delighted with the overall quality of the program, the engagement of the many attendees who asked several thought provoking questions, and the directions to be taken now that the Lancet Commission recommendations have been made.  Our involvement in Global Surgery will be taking on even greater importance as we move forward towards our next Strategic Plan in the Department of Surgery.  I do hope many of you will take advantage of all the offerings that will be available to you.

Sincerely,

James T. Rutka, MD, PhD, FRCSC
