Best Practices in Surgery
As the Department of Surgery enters its final year of implementation of its Strategic Plan, I am proud to reflect back on the immense strides made in Quality and Best Practices (Q/BP) under the tireless leadership of Dr. Robin McLeod.  In itself, 2016 was a busy year for Dr. McLeod and her committee, thus I should like to speak to some of the initiatives in our final column of the calendar year.
First and foremost, the Department of Surgery Q/BP program has been rebranded as Best Practices in Surgery.  This program now has a new website ( and monthly newsletter. Both communication pieces act as a central repository for U of T’s clinical practice guidelines, quality-related research, and events that may be of interest to those in the quality improvement domain. In the spirit of multi-disciplinary collaboration, the Q/BP Committee now includes representatives from Nursing and Anesthesia as well.
One of the Committee’s most exciting endeavours this year has been the Patient Experience Workshops, led by Dr. Erin Kennedy. The committee hosted three patient workshops with attendance from 30 surgical patients who had received their care at a U of T affiliated hospital across all Divisions, and 20 healthcare professionals including surgeons, anesthesiologists, residents, and nurses who are directly involved in surgical patient care. Core themes emerging from this workshop featured an increased need for discharge planning and enhanced physician-patient communication. The Committee is currently using these data to create specific initiatives that will be implemented across the hospitals.
The Committee has also been focusing its effort on the development of several clinical practice guidelines to be implemented in 2017. Guideline topics include Enhanced Recovery for All Surgical Specialties developed in collaboration with the Canadian Patient Safety Institute and Health Quality Ontario; Surgical Site Infection Prevention developed in partnership with the Antimicrobial Stewardship Program; Post-operative Wound Care developed in partnership with the Toronto Central CCAC; and Preoperative Fasting developed in partnership with the Department of Anesthesia.
Successful ongoing projects include the extensive analysis of readmission rates for index procedures across all surgical specialties, and the second year of a Quality Improvement Resident Curriculum.
I know of no program across Canada that is devoting such significant emphasis and effort on Q/BP studies and guidelines in Surgery.  In that way, we are all indebted to Robin McLeod for her leadership and insights into making this the best program of its kind in the country.
