MESSAGE FROM THE CHAIR

Competency by Design
 
By now, most of you have become quite familiar with the terms Competency Based Medical Education (CBME) and Competency by Design (CBD).  CBD is the new educational paradigm that is being spearheaded by the Royal College of Physicians and Surgeons.  Over the next 5 years, all surgical training programs across the country will be required to demonstrate proficiency in competency-based residency training.
 
We are very fortunate in the Department of Surgery at the University of Toronto that the Division of Orthopedics took the lead in CBME several years ago when it established its Royal College approved Competency Based Curriculum (CBC).  The CBC was a bold experiment in resident education that met with considerable acclaim worldwide.  Special recognition of this achievement must be given to Richard Reznick, Ben Alman, Bill Kramer, Markku Nousiainen, and Peter Ferguson who conceived the notion of the CBC, and who implemented it successfully together with the entire Division of Orthopaedics.  The Orthopaedics CBC emphasizes outcomes-based learning and consistent, in-depth skill evaluations.  In the upcoming CBD programs, resident evaluation will now be assessed by Entrustable Professional Activities or “EPA’s”.  These are tasks in the clinical setting that a supervisor can delegate to a resident who has demonstrated sufficient competence. 
 
The roll-out of CBD by the Royal College will occur step wise across different specialties.  For example, Otolaryngology and Medical Oncology began their implementation and field-testing in July 2016.  In our Department, several Program Directors have now begun to develop competency-based assessment tools aided in part by the Specialty Committees of the Royal College, and also the Postgraduate Medical Education (PGME) office at the University of Toronto.  One of the desired goals of the competency-based assessments is to identify residents who may be experiencing training difficulties at an earlier stage such that a carefully planned remediation program may be put in place for progression to the next level.
 
This year, the Surgical Foundations program for PGY1’s led by Ron Levine, Director of PGME in the Department of Surgery, is working towards the establishment of EPAs for residents across all specialties.  In the next academic year, the Division of Urology will be implementing its CBD program year with PGME and the Royal College.  We are indeed fortunate that the PGME office and staff already have considerable experience with the Division of Orthopaedics CBC, and are primed to assist all programs launch CBD with such tools as a resident assessment App which is currently under development.
 
The overarching intent of CBD is to shift learning into a more patient-centered domain where education is targeted towards modules of care that are repeated as required until competency is reached, and then surpassed.  In CBD, the role of the surgeon will go far beyond the traditional medical expert role, and lead to enhanced skills in communication, advocacy, leadership, and management. A comprehensive and learner-centered educational program will surely allow us to enhance the caliber of graduated residents who are able to provide high-quality patient care.
 
Are all the details worked out yet for CBD and its implementation across all surgical specialties?  The short answer is, no.  However, the Royal College will share best practices in CBD as it evolves with all groups leading to an iterative process that will continue to evolve and improve over time.  In advance of the adaptation of CBD programs in surgery, I would like to thank all Chairs, Program Directors, Faculty, and of course, residents, for your open-mindedness in making this one of the greatest opportunities in medical education to change residency training for some time.

New Appointments in the Department of Surgery, University of Toronto

Amit Atrey has been appointed as an Assistant Professor and Surgeon-Investigator in the Division of Orthopaedic Surgery at St. Michael’s Hospital in the Department of Surgery.  Amit completed his undergraduate degree in the United Kingdom and his medical degree at Guy’s and St. Thomas’ Hospitals, London, U.K.  He then did his orthopaedic residency in the UK. He completed a trauma fellowship in Hamburg, Germany; a Lower limb arthroplasty and trauma fellowship at St. Michael’s Hospital, Toronto; and a Clinical Fellowship in DePuy arthroplasty and soft tissue hip and knee at Warwick Hospital in the U.K. 






Nir Lipsman has been appointed as an Assistant Professor and Surgeon-Investigator in the Division of Neurosurgery at Sunnybrook Health Sciences Centre in the Department of Surgery.  Nir completed his undergraduate degree in psychology at the University of Toronto and his medical degree at Queen’s University.  He then did his neurosurgery residency at the University of Toronto.  He completed a clinical fellowship in stereotactic and functional neurosurgery at University Health Network.  Nir has an interest in the clinical and ethical implications of neuromodulation, and has been involved in the development of international guidelines for the use of surgery in psychiatric disease.  His research focuses on the development of novel neuromodulation strategies, which targets sites within the central nervous system using electrical stimulation or drugs, for neurologic and psychiatric conditions that are difficult to treat.

Lorraine Tremblay (General Surgery) has been appointed as President, Trauma Association of Canada for 2018-20.  Her appointment as president is a recognition of her contributions to trauma care in Canada over the course of her career.






Appointees to the Order of Canada
The Order of Canada is the highest honour that can be bestowed upon citizens of Canada, and are given to individuals who have demonstrated superior talent and involvement in fields such as the arts, science, politics, law, and medicine.  We are indeed fortunate that 2 individuals in our Department were awarded the Order of Canada as announced by the Governor General, the Honorable David Johnston on January 1st 2017.  Please help me congratulate the following surgeons:

Charles Tator (Neurosurgery) is Professor of Neurosurgery, and former Chair of the Division of Neurosurgery.  Charles has active research interests in spinal cord injury, and head injury/concussion.  Charles was previously appointed as a Member of the Order of Canada.   He has now been promoted through the Order to be an Officer of the Order of Canada.  



Michael Tymianski (Neurosurgery) is Professor of Neurosurgery, and Head of the Division of Neurosurgery at Toronto Western Hospital.  Michael specializes in cerebrovascular neurosurgery, and has research interests developing novel compounds to treat human stroke.  He has established a company which is testing a novel compound in clinical trials in stroke.  He is appointed as a Member of the Order of Canada.
