Changes to the Undergraduate (MD) Program at U of T: The Foundations Curriculum
 
As many of you know, a new curriculum is being launched in our medical school for the preclerkship years.  It is known as the “Foundations Curriculum”, and there will be numerous changes with which you should all become familiar.  The launch date is September this year.  A myriad of logistical issues have needed to be solved and implemented by the Undergraduate Medical Education (UME) Committee to roll out by this date.  I am pleased to report to you that the Medical School is well on its way. 
 
A variety of driving factors have instigated the need for adaptation in medical education. These changes have been extensively explored in the 2010 Carnegie Report, the Lancet Commission’s ‘Health Professions for a New Century’, and the Future of Medical Education in Canada Project. The major changes include a competency-based approach, integration of clinical sciences with public and community health, and flexibility to pursue individual interests.
 
Case-based learning in small group settings will allow for integration with the relevant clinical skills, as will an increase in the amount of available e-learning materials. Students will be able to learn at their own pace while pursuing individual interests that complement their medical training, such as performing research in the MD/PhD program for Comprehensive Research Experience for Medical Students Program (CREMS). There will be more frequent and lower-stake assessments to ensure consistent individual feedback, including emphasis on leadership, ethics, and inter-professional skills.
 
After extensive review with the Undergraduate Education Committee, the new clerkship program in Surgery will result in a revised course format and on-call evaluation tools. Under the Foundations Curriculum, students will complete 1 week of the Prelude to Surgery Program; 3 weeks of General Surgery, 3 weeks of sub-specialty choice; 1 week of exam preparation. We believe that 3 weeks for sub-specialty rotations will provide a more comprehensive learning experience and accurate assessment. To this effect, an on-call evaluation form has been developed for General Surgery, Orthopaedic Surgery, and Neurosurgery, effective January 18th. The forms are available on the portal and will be filled in by the staff or resident on call, with the option for supervisors to review at their own discretion. The rationale for these changes is to reduce the amount of administrative work during the on-call period, while still being able to provide valuable feedback for the clerks. These guiding principles will result in an active learning approach in which students can develop their identities as professionals within a changing environment, such as increased use of technology and attention to the healthcare system.
 
On average, there will be a slightly greater requirement for each clinical Department with the launch of the Foundations Curriculum.  I would like to take this opportunity to thank all of you for your tremendous commitment to the education of our medical students, and your willingness to participate in the changes that lie ahead.

The Department of Surgery welcomes Sybil Rose Devon-Lerner born January 24, 2016, at 6 lb, 6 oz. Congratulations to Karen Devon and Jordan Lerner!

Steve McCabe led a multidisciplinary team toward the first hand transplant in Canada
Dr. Steven McCabe and a multi-disciplinary team successfully completed Canada’s first transplant of the upper limb.  The procedure lasted approximately 14 hours during which a team of 18 surgeons attached the forearm and hand matched from a donor to a patient who had been evaluated as a suitable candidate. This required the cooperation of a variety of different surgical disciplines from a number of hospitals and the University of Toronto.

Continuing Appointment Reviews (CARs)
The following Continuing Appointment Reviews (CARs) took place in January under the leadership of Ori Rotstein, Associate Chair, Department of Surgery.  
 
Shady Ashamalla is an Assistant Professor and Surgeon Educator with the Division of General Surgery, Department of Surgery, University of Toronto and a faculty member and Interim Division Head at the Odette Cancer Centre,  Sunnybrook Health Sciences Centre.   He is a Minimally Invasive Surgical Oncologist with an interest in Lower Gastrointestinal Surgical Oncology with a clinical focus on the Minimally Invasive approach.  His academic interests are in Surgical Simulation and Knowledge Translation. Shady obtained his undergraduate Honours degree from the University of Guelph in Biomedical Sciences followed by a Master of Science degree at Queen's University. He then returned to Toronto and completed his medical degree followed by a residency in General Surgery. After completing his training in General Surgery he did a fellowship in Minimally Invasive Surgery followed by another in Surgical Oncology. From left to right:  N. Cobourn, S. Ashamalla, A. Jas Chahal is an Assistant Professor with the Division of Orthopaedics, Department of Surgery, University of Toronto and a faculty member at Women’s College Hospital and Toronto Western Hospital.  He is a fellowship trained orthopaedic sports medicine surgeon with an interest in shoulder, hip and knee arthroscopy, as well as, articular cartilage restoration.  Dr. Chahal has been closely involved in the creation of the Cartilage Initiative and Restoration Centre at the University of Toronto (CIRCUT) in order to create a center of excellence for the management and study of patients with articular cartilage lesions. This program will work closely with UTOSM and the UHN Arthritis Program.   Jas obtained his medical degree from the University of Manitoba and then did his Orthopaedic residency at U. of T.  This was followed by a Sports Medicine Fellowship  at U. of T. and then Rush University Medical Center.  He has since obtained his MSc in Clinical Epidemiology, Health Policy, Management and Evaluation at U. of T.; an EMBA, Executive Master of Business Administration at the University of St. Gallen, Switzerland; and an MBA from the Rotman School of Management, U. of T. Laura Snell is an Assistant Professor with the Division of Plastic and Reconstructive Surgery, Department of Surgery, University of Toronto, and a faculty member at Sunnybrook Health Sciences Centre. Her main clinical interest is in the reconstruction of oncologic ablative defects, particularly breast reconstruction. Her clinical goal is to establish a leading breast reconstruction program at Sunnybrook. She is also interested in the surgical treatment of patients diagnosed with melanoma.  Laura received her MD from Western University in London is a graduate of the Plastic Surgery Residency Program at the University of Toronto. This was followed by a 2 year research and clinical fellowship in reconstructive microsurgery at Memorial Sloan Kettering Cancer Centre, New York.   During this fellowship, Laura also completed a Masters of Science in Biostatistics at Columbia University.  Laura is the Journal Club Coordinator in the Division of Plastic Surgery at the University of Toronto. Her research interests include the evaluation and management of patient expectations following breast reconstruction, and how better patient education can subsequently improve postoperative satisfaction. Fayez Quereshy is an Assistant Professor with the Division of General Surgery, Department of Surgery, University of Toronto, and a faculty member at the University Health Network.  He is a graduate of the University of Toronto Medical School and General Surgery Training Program. During his General Surgery training Dr. Quereshy entered the Surgeon Scientist Program and was awarded a Masters of Business Administration (MBA) from the Joseph L. Rotman School of Management at the University of Toronto.  He then went on to complete a Fellowship in Surgical Oncology at the University of Toronto and Robotic Surgery Training in Hong Kong. His clinical practice is centred on minimally invasive and colorectal surgical oncology. Fayez’s research will focus on Hospital Administration and specifically Operations Management and Process-level Evaluation and Optimization and Outcomes.  From left to right: A. Okrainec, F. Paul Karanicolas is an Assistant Professor with the Division of General Surgery, Department of Surgery, University of Toronto and a surgical oncologist at the Odette Cancer Centre, Sunnybrook Health Sciences Centre.  He completed medical school and residency in general surgery at the University of Western Ontario. During his residency, Dr. Karanicolas entered the clinician-investigator program and completed a PhD thesis at McMaster University.  Paul then completed a clinical fellowship in Surgical Oncology at Memorial Sloan-Kettering Cancer Center in New York.  His  surgical practice is devoted to patients with hepatobiliary, pancreatic and gastrointestinal cancers. Paul is actively engaged in clinical research which  involves writing and conducting clinical trials for patients undergoing surgery, particularly in the area of gastrointestinal oncology. He is also focused on improving the methods to collect and interpret data on quality-of-life in patients with cancer. In addition, he is actively involved in improving the incorporation of evidence-based care into surgical practice through the development of clinical guidelines and teaching.  From left to right:  C.
