Care Partners: Bridging Families, Clinics, and Communities to Advance Late-Life Depression Care
A promising approach to improving late-life depression care is through the systematic involvement of community-based organizations, family members, and primary care clinics that interact with older adults. The Archstone Foundationis funding the Depression in Late-Life Initiative Care Partnersproject and is accepting letters of inquiry from eligible community-based organizations and primary care clinics located in California through January 16, 2015.
